The Secret of the Spring side quest is tucked away in a remote corner of the Forsaken Quarry region. Getting to the starting point requires some tricky navigation. Refer to the map below to locate the exact marked area in the Northeasternmost part of the quarries, near the border wall.
Battling through several packs of Gnarled Iratha enemies, look for a winding path leading up into the cliffs. At the top sits an abandoned camp with a tall ladder propped against the rear rock face. Climb to the top to find the "Discarded Note" which initiates the quest.
Read through the full note to add Secret of the Spring to your quest log. The lengthy text contains clues in the form of a riddle to solve.
The discarded note‘s flowing script reads:
Beneath the full moon‘s glow
A hidden wonder grows
Where icy waters flow
And steam rises to thaw winter‘s snow
Seek it out and make your presence known
Then precious loot shall be your own
Clearly this poem refers to locating a hot spring. The line about "making your presence known" likely means we must perform an action once discovering it. As for its location, we know from the environment description to search this snowy, mountainous area.
After some searching, an isolated hot spring can be spotted in a nearby valley. Marked by a thin rising steam, crystal clear waters bubble up from underground heated sources into a perfect circular pool.
Approaching carefully while dealing with 2-3 plague bats, make your way down into the secluded valley toward the hot spring. Equip the "Wait" emote to your quick action wheel if it isn‘t already slotted. The note specifically hinted we would have to "make our presence known" once arriving.
Fend off any last enemies in the area then walk up to the magical waters. Face the correct direction and activate your Wait emote.
Performing the Wait emote triggers a special scripted event. A notification pops up stating "You feel the ground shift underfoot…" Seconds later an ornate chest spawns half-buried in the soil along the pool‘s edge. Interact with the chest to complete the quest and claim two worthwhile rewards.
Opening this secret chest awards 4620 XP to boost you closer to the next level and 1000 gold coins for much needed supplies/upgrades. With no other objectives the Secret of the Spring side excursion ends here. The multi-step puzzle makes locating this hidden oasis quite an achievement!
